Life after Discharge: Seniors with Multiple Chronic Illnesses

Many seniors have family members that live to far away to help them with their daily needs. Having someone in the home to help with cooking, cleaning, medication reminders, and personal hygiene is often all that is needed to help keep at risk seniors from being readmitted into the hospital during those critical first few weeks after discharge.
