OVERVIEW OF COPD 

COPD, or chronic obstructive pulmonary disease, is a broad term that is used to describe progressive lung diseases that block the flow of air into the lungs. It includes emphysema, chronic bronchitis, and refractory asthma.  

Having COPD makes it difficult to breathe and perform some daily functions of living. It can cause coughing, fatigue, chest tightness, respiratory infections, and other symptoms that can impact a senior’s quality of life. COPD is also one of the top 5 causes of hospital readmissions, which is why we have created a specialized program to keep these seniors safe and at home.   

 

 

WHAT’S COVERED IN OUR STROKE SPECIALTY PROGRAM 

Our COPD care program helps our caregivers identify the needs and preferences of each senior with COPD or similar respiratory conditions. We coordinate care with health professionals and family members to ensure that seniors remain happy and at home. 

 

We can help with:

  • Monitoring weight and fluid intake 
  • Nutrition management and meal preparation 
  • Mobility and safety monitoring 
  • Medication reminders 
  • Transportation to doctor’s appointments 

 

Our caregivers are trained in:

  • Understanding COPD and its effect on seniors 
  • Fall risk and prevention 
  • Proper techniques for transferring and positioning seniors 
  • Assisting with home exercises 
  • Care coordination with healthcare professionals
